We’ve developed the following practice policies to provide structure and clarity for our work together. Please initial each policy indicating that you have read and understand it. If you have any questions please feel free to contact our office.
All of our work together is confidential unless I think there is a risk of harm to yourself or another person. Please be aware that as nurses we are Department of Children and Families mandated reporters. This means that if we suspect a child may have been abused or neglected, we are required by law to report those suspicions to Family Services within 24 hours.
We accept Medicaid. All other patients pay for their visit at the time of service. If the service is eligible to be submitted to an insurance carrier, our staff can give you a superbill to submit to your insurance company. Because all insurance plans operate differently and have different schedules of benefits, it will be important to speak with your carrier before receiving services in order to obtain the most clear understanding of the reimbursement. Trillium cannot guarantee the monetary amount, if any, that will be reimbursed to you by your insurance carrier for services rendered.
The cost for a new patient visit is $245.00
Follow-up visits are $195.00
Why this approach?
We want to build a relationship with you, not your insurance company. This model helps us spend more quality time with you and be more comprehensively committed to your overall health and well-being. Our appointment times range from thirty to ninety minutes, based on your needs. You’ll leave your appointment knowing that we have done our best to address all your concerns.
Laboratory Fees: We aren’t directly affiliated with any labs, although we will provide the needed order forms and test kits for the tests you need, and direct you to lab services we’ve found to be reliable. The labs will bill you or your insurance directly for their services.
We will be happy to give you contact information for the various lab customer service offices, and the insurance billing codes so you can find out what your lab costs will be. Many standard tests are usually covered by insurance, but our staff can’t answer questions about your specific plan.
No Show/Late Cancellations/Late for Appointment Policy:
We require 24 hours notice if you need to cancel an appointment. No-shows or late- cancellations will result in a charge billed to you personally (health insurance does not pay for cancelled or missed appointment charges.)
Because we understand that emergencies happen, every patient will get one “free pass” per calendar year. That means you can miss one appointment without the required 24-hour cancellation notice and not be charged.
Repeated no-shows will result in termination of our provider-patient relationship.
Our time together is the most important element of our health care partnership. If you arrive 15 minutes late or more to your appointment you will likely be asked to reschedule. If we get behind schedule, we will make sure that you get the full time allotted for your appointment.
Medication Refills and Controlled Substances:
Response to a request for medication refills may take 48-72 hours. Please plan ahead; you should contact our office at least three (3) days before your medication is due to run out. Medication refills should be requested by calling our Office Administrator at 802-654-3562, emailing her at Shannon@trilliumvt.com, or in person during your appointment.
Controlled substances (such as ADHD medications or medications for anxiety) are strictly managed. A urine drug screen may be required before a refill is given.
In case of a health emergency or if you have a reaction to a medication please call 911 or go to your local emergency department or urgent care center.
